How to read your insurance card

Policy ID Number
This number is typically the largest number on the front of the card, next to the policy holder's name.

Group Number
This number should appear on the front of the card, usually below the Policy ID Number.

Insurance Phone
This is a toll-free number that is most often located on the back of the card.

Your ENBREL prescription should be written for one of the following doses:
50-mg/mL SureClick® autoinjector
50-mg/mL single-use prefilled syringe
25-mg multiple-use vial
25-mg/0.5-mL single-use prefilled syringe
And one of the following dosing schedules:
Once weekly
Twice weekly
Twice weekly for 3 months, then once weekly
