Program Details

*Eligibility: Open to patients with commercial prescription insurance coverage for ENBREL. Not open to uninsured patients or patients receiving prescription reimbursement under any federal, state, or government-funded insurance programs (for example, Medicare including Medicare Part D, Medicaid, etc.) or patients who live in Massachusetts (or where prohibited by law). Restrictions, including monthly maximums, may apply. Offer subject to change or discontinuation without notice.

^†Each time you fill your ENBREL prescription, you will receive 50% off your out-of-pocket costs or pay no more than $10 per month—whichever helps you more.

Program provides up to $4,000 of assistance per patient for each 6-month period.^‡ Patient is responsible for costs above these amounts.

You may use your card each time you fill your ENBREL prescription.

Your participation in the program may be renewed every 12 months by visiting EnbrelSupport.com or calling 1-888-4ENBREL.
